*Job Summary: *
Provides oversight and management of the Document Tracking Specialists in relation to their functions of processing and tracking physician signed documentation for Home Health patients. Responsibilities include, but are not limited to ensuring the functions of faxing information to physicians, sorting, scanning and attaching physician signed documentation into Electronic Medical Records (EMR) system are done timely and accurately. May be required as needed to assist directly in the below operational activities of the Document Tracking Specialist to ensure team maintains timely processing.
*Your responsibilities: *
Ensures that the following day-to-day functions and duties of the Document Tracking Specialist team are completed
within specified parameters:
Provides direct supervision, training, coaching, support and development of DTS team.
Coordinates activities of DTS department with immediate supervisor and other company level executive leadership.
Mails, faxes, or prepares hand delivery of documents requiring a physician signature and complies with physician preference for transmission method.
Responsible for data entry associated with Home Health episodes for tracking documentation, orders sent, orders received with signatures, and ensuring accuracy of data entry in accordance with daily and weekly reconciliation procedures.
Reports daily any items outside of agency established timely parameters and coordinates with Supervisor and agency level care team staff to comply with agency timeline standards.
Ensures orders, and episode discharge summary (at discharge, re-certification, and transfer) are sent to physicians timely, according to agency standards.
Secures the necessary physician signatures and clinical staff signatures for the clinical documentation as required by local, state, and federal requirements.
Keeps the Home Health Director informed on outstanding order statuses for the assigned care team.
Provides training to Document Tracking Specialist staff on professional customer service to physicians, physician office personnel, agency staff, sales partners and other referral sources.
Tracks and provides follow-up actions for any outstanding physician signed face-to-face encounter / visit documents per agency standards.
Maintains a safe and clean work area and adheres to facility/corporate safety standards.
Ensures that the DTS team maintains positive relationships and rapport with co-workers, associates, patients and their family members and community personnel.
Adheres to communication and accountability guidelines established by local and regional administration.
Adheres to established confidentiality standards as required by Brookdale and HIPAA concerning community and patient information.
Monitors reports and other KPI"s to ensure DTS department is operating in accordance to company standards and expectations.
Required skills and qualifications:
High School Diploma required. Associate"s or Bachelor"s degree preferred.
Experience with speaking with physician offices and obtaining signatures
A minimum of two years of experience in a related health care office setting.
One year home health office experience preferred. Proficiency with computer use and software applications including Microsoft Office and EMR software preferred.
