Brookdale Healthcare Services is looking for a Home Health Regional Intake Registered Nurse to provide services in the Brentwood area. Join a dynamic team of health care providers focused on health, wellness and prevention for all our patients.


Your responsibilities:*

  • Manage the intake process for home health, set up patient account information, and facilitate face-to-face physician visits

  • Manage all assigned intake/admission EMR work flow for Home Health patients (initial referrals, physician verification, etc.) per assigned care team

  • Provide care team with daily reports regarding pending admissions, new admission and outstanding (open) items

  • Obtain initial payer source eligibility verification and coordinate necessary follow-up to communicate patient"s expected out-of-pocket expense

  • Conduct or assist with special projects as requested by the administrator and other Brookdale managers

Required skills and qualifications:

  • Associate"s or Bachelor"s degree in nursing; current Registered Nurse licensure

  • Two years office experience, preferably in a home health agency setting

  • Proficiency with computers and software applications, including Microsoft Office

  • EMR software experience highly preferred
