- Evaluates and assesses patient"s psychosocial status to identify problems that may affect the patient"shealth status.
*Assists physician and home health care team members in evaluating patient"s social, emotional andeconomical status, and identify community resources and other possible resources that can aid patient andfamily in treating and coping with identified problems and issues.
*Assesses psychosocial and emotional status and initiates Baker Act if needed, for placement in an impatientpsychiatric family.
*Makes referrals to Adult Protective Services, as needed.
*Sets realistic goals for patient and incorporates the goals in the plan of care.
*Provides counseling or intervention to patient according to the plan of care.
*Prepares clinical and progress notes to treatment provided to patient and submitted in a timely manner.
*Evaluates effectiveness and outcomes of care provided to patient.
*Participates in planning for discharge of patient.
*Assesses and screens patients that are suitable for placement in a long-term care or assisted living facilitywhen no longer feasible to care for patient at home due to patient"s physical or mental state.
*Provides treatment in compliance with the agency policies and procedures and federal and state regulations.
Required skills and qualifications:
Masters Social Worker degree from a school of social work accredited by the Council on Social Work Education (CSWE)
At least one year of experience
Active license in good standing within the state of practice
