Brookdale Healthcare Services is looking for a Home Health Intake Coordinator to provide services in the Phoenix area. Join a dynamic team of health care providers focused on support and comfort for all our patients.

Your responsibilities:

  • Manage the intake process for hospice, set up patient account information, and facilitate face-to-face physician visits

  • Manage all assigned intake/admission EMR workflow for hospice patients (initial referrals, physician verification, etc.) per assigned care team

  • Provide care team with daily reports regarding pending admissions, new admission and outstanding (open) items

  • Obtain initial payer source eligibility verification and coordinate necessary follow-up to communicate patient"s expected out-of-pocket expense

  • Conduct or assist with special projects as requested by the administrator and other Brookdale managers

Required skills and qualifications:

  • High school diploma or GED; Associate or Bachelor"s degree preferred

  • Two years of office experience, preferably in a hospice agency setting

  • Proficiency with computers and software applications, including Microsoft Office

  • EMR software experience highly preferred
