Brookdale Healthcare Services is looking for a Hospice Chaplain to provide services in the Milwaukee area. Join a dynamic team of health care providers focused on support and comfort for all our patients.
Assure spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner
Provide direct spiritual care to patients and families/caregivers
Serve as liaison and support to community chaplains and spiritual counselors
Document direct services and ongoing communication with community chaplains and spiritual counselors
Recruit community chaplains, spiritual counselors, and spiritual care volunteers to meet patient and family/caregiver needs by developing community contacts and offering education through congregations about hospice care
Provide for funeral or memorial services for patients as requested
Required skills and qualifications:
Graduate of accredited seminary or school of theology (BD, M.Div. or equivalent theological degree), licensed ordained minister or equivalent experience
Experience working with death and dying, individuals/family/caregiver
Ability to accept different lifestyles, cultures, beliefs and values
Hospice experience preferred
Knowledge of and commitment to hospice philosophy of care; hospice experience preferred
